I think my spouse took a second mortgage on our home without telling me about it. How can I find out if he did or didn't?

That would be difficult for him to do, but not impossible.  You have a couple of options here.

A mortgage would be recorded in your county court house.  This is completely accessible to anyone from the general public to see if they know where to look.  You could go to your county courthouse and attempt to search for recorded liens against the home.  Should you have a helpful clerk you may be able to accomplish this.

A simpler solution may be to look up title insurance or settlement agent in your local phone book.  For a nominal fee they can have a professional searcher find all liens recorded against the property.

There are a few ways. You could contact a local title companyand have them doa title search for you which will reveal all liens, lenders and dates of origination.

You can go to your local courthouse and pull the records yourself(or with their help). Some courthouses charge a very nominal fee for this.

You can also pull your credit if you think he put your name on it without your permission. You weren’t clear if he did this in his own name or both.

If you’re name is on the title from the first mortgage, there really isn’t any way for him to get a 2nd mortgage without your signature so if he did, you’re now getting into possible fraud. I would go to courthouse and then contact an attorney for the proper legal advice.
