I have a Seperation Agreement form signed by myself and my ex along with a quitclaim deed for the matramonial home and signed with two witnesses. where do I take this deed to get transfer completed.

The deed needs to be recorded in the public land records of the county in which the property is located. These are called different things in different states and most counties have the information online.
